Applications for Contracts Under $400K
Complete the following applications:
- Quick Express Application
- Credit Release Form - one per owner
- Personal Financial Statement - one per owner (only complete if you are applying for a $700K bond line)
If this is for a Bid Request please enclose:
- Invitation to Bid and Bid Specifications
- Bid Form
- Special Bond Forms (if required)
If this is for a newly contracted project, please enclose a copy of the award letter and the contract or purchase order in your fax submission.

*Companies less than 1 year in business are limited to a $100,000 line.
Applications for Contracts Over $400K
Complete the Contract New Account Data Form.
Attach a copy of your business financial statements for the last three years (CPA prepared is preferred).

Complete the following applications:
- Bid Bond Request Form - One per bond
- Performance Bond Request Form - One per bond
